Three surviving ledgers describe one revision applied to a sealed record. The revision carries no author.

00 · Incident

A sealed record was altered. The alteration is present in the Archive. The author is not.

Reconstruction is possible only by placing the three surviving ledger stamps on a single timeline. Read the stamps, not the narrative.

01 · Access Trace · NX-05-013/A

STAMP 02:13:00 — Session R-05 opens the sealed record in write mode.

Session credential: UNLABELLED. Terminal: unmapped. No authorization token is attached to the request.

The trace records the open. It does not record permission.

02 · Revision Ledger · NX-05-013/B

STAMP 02:26:00 — Revision committed to the sealed record by session R-05.

Author field: EMPTY. Reviewer field: EMPTY. Justification field: EMPTY.

The ledger confirms the write completed. It does not confirm it was permitted.

03 · Authorization Register · NX-05-013/C

STAMP 02:39:00 — Write authorization for session R-05 is granted and countersigned.

The register is valid, complete, and correctly formed. It is also the last stamp in the sequence.

Compare the three stamps in order. The fault is not in any single ledger — it is in their order.

04 · Directive

Return to the Terminal and transmit the ordering fault the reconstruction exposes.
